Sarah de Souza, who was crowned as Miss Massachusetts USA 2021 during a gala pageant, held at the Lowell Memorial Auditorium in Lowell on June 13, 2021, became the official representative of Massachusetts at Miss USA 2021. Miss Massachusetts USA 2021 Sarah de Souza succeeds Miss Massachusetts USA 2020 Sabrina Victor.

The Brasileira-Bostonian beauty has studied interior design from Suffolk University in Boston. The beauty queen loves painting and is proficient in digital design as well. While Sarah de Souza was crowned as Miss Massachusetts USA, Laura Dos Santos was sashed as the first runner-up, Skarlet Ramirez became the second runner-up, Samantha Vocatura was chosen as the third runner-up, Ashley Cunningham became the fourth runner-up and Olivia Alexandria was selected as the fifth runner-up.
